Acute and Community CDR Input Standard (Version 2.6.1)

The standard aligns with other Ontario specifications as well as pan-Canadian and international document standards and specifications.

The acCDR Input Standard version 2.6.1 is now available with revised scope for data contribution. Please refer to the release notes included in the downloadable package for guidance to data contributors as to which version of the input standard to use.

Scope

The acCDR provides access to provincial acute and community care data, improving the timeliness of care decisions by reducing duplicate tests and procedures and better supporting care transition points. It also supports medication administration data contribution to Ontario Digital Drug Repository (DHDR) using HL7 v2 standard.

Starting with clinician-identified priority data, the ConnectingOntario ClinicalViewer will leverage health care organizations and provincial digital health repositories to provide access to the following data: acute (e.g. discharge summaries, consultation reports, emergency department reports, visits and encounters, Ontario Health atHome reports, diagnostic imaging, and lab results). Providers also have access to data from other provincial repositories including Diagnostic Imaging Common Service (DI CS), Ontario Laboratories Information System (OLIS) and Digital Health Drug Repository (DHDR) through ConnectingOntario ClinicalViewer.

Contributing health service providers will supply these electronically captured data types that can be transmitted in an HL7*-compliant format.

*HL7 is the primary standard used to move information from one system to another within and between health care facilities.

Status

Acute and Community CDR Input Standard v2.6.1 is a Ministry of Health approved specification, under Ontario Regulation 329/04, s. 26 – 34.

There is an expectation that there will be further revisions to this specification, as the function of medication administration data in the Ontario Digital Drug Repository (DHDR) matures.
