Ontario eForms HL7® FHIR® SDC Implementation Guide v1.0.0 - Ballot
The Ontario eForms HL7 FHIR SDC Implementation Guide is created to reduce administrative burden on clinicians, streamline processes, eliminate redundancy, enhance efficiency and provide faster service to patients. eForms implementation guide will provide a standard framework for consistently defining, rendering, and processing digital forms in a way that can be implemented in user-friendly interfaces. Forms builders will be able to define how forms look, how they behave and how other systems use the forms in the same way.
Scope
The Ontario eForms HL7 FHIR SDC Implementation Guide defines how standardized, structured electronic referral (eReferral) forms are designed, rendered, and processed using the HL7 Structured Data Capture (SDC) framework.
This guide focuses on form rendering and behavior — specifically how FHIR-based forms are displayed, completed, and exchanged across Ontario’s digital health ecosystem. The eForms Implementation Guide applies to the following components:
- Definition and implementation of Ontario FHIR resource profiles and extensions within eForms Designer.
- Use of standardized value sets and code systems.
- Rendering and presentation of Questionnaires across various Referral Management Systems in Ontario.
- Integration with launch contexts and attachment handling.
Status
The provincial open review period starts October 23, 2025, and runs to November 21, 2025. Download the comment spreadsheet to provide feedback.
This implementation guide is for trial use and is subject to a formal review and approval process.
FHIR is an evolving trial-for-use standard, so additional changes by HL7 are expected. We will continue to monitor developments and assess any potential impacts and publish updates as they emerge.
