Ontario Integrated Assessment Record (IAR) HL7® FHIR® Implementation Guide v1.0.0-ballot
The Integrated Assessment Record (IAR) provides a central repository for clinical assessment data collected from multiple Health Information Custodians (HICs), including assessor; private/public hospital, and health service providers (community care services, mental health addiction services). Assessment information, collected at the point of care, can be shared within the client’s circle of care in a timely, secure, and privacy-protected manner. As a client moves through the continuum of care, consented assessments are accessible by the clinicians providing care. The IAR enables collaborative care planning as well as enhanced communication between providers with the goal of promoting high quality care for clients in the community.
Scope
The IAR IG provides guidance and framework for submitting completed assessment forms to IAR’s central repository. It supports implementers of digital health assets that will use HL7® FHIR® to enable electronic sharing of community assessments among HICs and the central IAR repository. The guide specifies a set of rules for using FHIR® and FHIR® resources to publish and retrieve assessments to/from IAR repository.
The guide includes:
- Intended use, data elements and terminologies
- considerations for system-to-system integration using FHIR®
- conformance expectations
Status
The provincial open review period starts March 26, 2026, and runs to April 30, 2026.

This implementation guide is for trial use and is subject to a formal review and approval process.
FHIR is an evolving trial-for-use standard, so additional changes by HL7 are expected. We will continue to monitor developments and assess any potential impacts and publish updates as they emerge.
