The connectivity strategy defines the path to connecting a patients’ health information to their care team. Its focus is identifying the sources of digital health information in Ontario, making it sharable with through integration with electronic record assets, and delivering it to the people who need it. It outlines current and futures states of the electronic health record and the transition between the two. And it describes how health care information will be connected to create an integrated electronic health record – one that provides a lifetime record of an individual’s health history.

Based on extensive consultation and contributions from the Ministry of Health and Long-Term Care and a wide-range of stakeholders including clinicians, the strategy was designed to inform investment and integration decisions. It illustrates how local and regional health information technology solutions need to integrate with provincial assets to become part of Ontario’s electronic health record. Major themes include consistency and reuse, health care industry trends, health informatics standards, and common specifications.

The strategy identifies provincial assets that stakeholders can combine with local assets to address their own health care challenges, as well as services provided by Ontario Association of Community Care Access Centre, Cancer Care Ontario, and Ontario Telemedicine Network that could be leveraged to address broader provincial needs. It emphasizes reuse, standardization, and leveraging investments made to date, while providing a transition plan to the future state of the electronic health record.
