Acute and Community CDR Input Standard
The Acute and Community clinical data repository (acCDR) supports patients by providing care teams with a central point of access to an up-to-date, integrated care record. The Acute and Community CDR (acCDR) input standard defines the messaging format, data content, and terminology for contributing clinical data to the CDR. It may also be used by health service providers to implement OntarioMD’s health report manager for organizations contributing to this system and acCDR.
The standard aligns with other Ontario specifications as well as pan-Canadian and international document standards and specifications.
The acCDR Input Standard version 2.5.1 is now available with revised scope for data contribution. Please refer to the release notes included in the downloadable package for guidance to data contributors as to which version of the input standard to use.
Scope
The acCDR provides access to provincial acute and community care data, improving the timeliness of care decisions by reducing duplicate tests and procedures and better supporting care transition points.
Starting with clinician-identified priority data, the ConnectingOntario ClinicalViewer will leverage health care organizations and provincial digital health repositories to provide access to the following data: acute (e.g. discharge summaries, consultation reports, emergency department reports, visits and encounters, LHIN reports, diagnostic imaging, and lab results). Providers also have access to data from other provincial repositories including Diagnostic Imaging Common Service (DI CS), Ontario Laboratories Information System (OLIS) and Digital Health Drug Repository (DHDR) through through ConnectingOntario ClinicalViewer.
Contributing health service providers will supply these electronically-captured data types that can be transmitted in an HL7*-compliant format.
*HL7 is the primary standard used to move information from one system to another within and between health care facilities.
ConnectingOntario ClinicalViewer Data Mapping Guide
This mapping guide provides an outline of ConnectingOntario ClinicalViewer and how the data elements in the input specification of the corresponding digital health repositories are mapped to the front end data elements, as well as document mapping to allow for categorization to filter and display in the appropriate front end components.
The ConnectingOntario ClinicalViewer Data Mapping Guide version 1.4 is now available with the updated information for acCDR Release 10.
